<International Circulation>: We have seen at this meeting a cooperative effort to exchange ideas between China and ACC. What avenues of cooperation can we expect for the future and why is this so important? Professor Ralph Brindis: it fills me with great honour representing the ACC here in Beijing and to be able to work with Professor Hu DaYi who I think is a terrific visionary for the treatment of cardiovascular disease (CVD) here in China. By working with Professor Hu the ACC and himself have devised what I think is a terrific scientific curriculum which I am hopeful as he will make inroads in the treatment of CVD in China. We are very excited about the prospect of developing future collaborations in science, education and in evaluating quality so that we can translate what is evidence based clinical guidelines into practice for the people of China.

Brindis教授:我非常荣幸能够代表ACC来到长城会,跟胡大一教授交流。我认为胡大一教授对于中国心血管疾病的治疗具有远见卓识。本次大会的内容非常精彩,胡大一教授参与了大会内容的设置。我预祝,胡大一教授在未来能够在中国心血管疾病的治疗方面取得突破。ACC对于未来跟中国同仁在科学、教育和医疗质量评价方面的合作充满期待,藉由上述各方面的合作,我们能够把以循证为基础的临床指南转化为中国的临床实践。 <International Circulation>: There has been a lot of discussion regarding on what is the ideal blood pressure level for diabetic patients. Please offer your opinion on what you think is the best measurement we should strive for? Professor Ralph Brindis: this was one the most important messages we received at the annual ACC meeting this March. I would say that sometimes that less is more. We have been thinking that it would be very excellent to have better control of blood pressure particularly in diabetic patients that this would positively affect cardiovascular outcomes. What we have learnt particularly with the ACCORD Study is that when you aggressively lower blood pressure not only did that not improve cardiovascular outcomes but it actually led to a modest increase in strokes and symptoms related to the increase in multiple medications patients were taking to achieve such low blood pressures. I think this is excellent news for both patients and physicians in that we should strive for blood pressure targets which are more reasonable and easier to attain and with the knowledge that it does not adversely affect out patient’s outcomes.
《国际循环》:糖尿病患者理想的血压水平是多少?对此争议颇多。您觉得理想的血压水平是多少? Brindis教授:这是我们在2010年3月召开的ACC年会上所得到的最重要信息之一。我想说的是,在某些情况下,多即是少。我们一直以为,将血压控制得更严格是非常好的,尤其是在糖尿病患者,严格控制血压能够改善患者的心血管转归。但是,我们目前知道,尤其是通过ACCORD研究,将血压降得过低不但不能改善患者的心血管转归,实际上还会轻度增加卒中风险,另外还会引起为达到更低血压而更多服药的症状出现。我认为,我们应当努力将血压降到更为合理且更容易达到的水平,同时要明确这一血压水平不会对患者的转归产生不良影响,我想这无论对患者和医生来讲都是个好消息。
